About support at home

From 1 November 2025, the Support at Home program replaced the Home Care Packages program across Australia. The new program streamlines how services are organised and makes care at home more personalised and easier to navigate.

Whether you currently receive support or are just getting started, Trilogy Care is here to help. We’ll explain what’s changed and what stays the same, map your goals to the new service categories, and help you plan and track your quarterly budget. If you self-manage, we’ll support you to choose and coordinate your workers while we handle safety, governance and claims. Your dedicated care partner will work with you to make the most of your care.

What´s included in the service?

By choosing our elder care service, you can rest assured that your loved ones are in capable and caring hands. We are committed to providing exceptional care that enhances the quality of life for each and every client.
How is Support at Home different from HCP?

While Home Care Packages offered a broad range of flexible services, Support at Home Program introduces a more defined service list, emphasising a wellness and reablement approach, focusing on doing with, not for ensuring supporting the maintenance of independence as you age.

It also introduces clearer co-contribution expectations, encouraging greater involvement and shared responsibility in care.

Support at Home features eight classification levels, with annual budgets ranging from around $11,000 up to $78,000, depending on assessment results and care needs. These budgets are paid out quarterly

Support at Home provides access to a wide range of services including personal care, allied health, home modifications, transport, and short-term care for recovery or end-of-life support.

You can apply through My Aged Care by calling 1800 200 422 or submitting a referral online. This is the first step to receive government-funded aged care under the new Support at Home program

The Support at Home program introduces several improvements over the current Home Care Package system:

More funding – The maximum annual funding will increase from $63,440 to $78,106
Care partners – A registered provider will help manage your care and services
Better pricing – No separate administration or management fees
Faster access – Wait times for funding will reduce to three months by 2027
Assistive Technology & Home Modifications – Up to $15,000 upfront for things like walking aids and bathroom modifications
Short-term support – Additional restorative care funding and new programs including the End-of-Life Pathway, AT-HM Scheme, and Short Term Restorative Care Pathway to help people remain at home in their final months
